Nurses' Notes
We would like you to be aware of State Mandated Immunization requirements for Middle School Students.
1. We are required to comply with state immunization mandates. We report immunization (shot) records to the state every year. Proof of the tetanus booster shot (Tdap) and a second varicella (chicken pox) shot is required in Grade 6, before your child enters seventh grade. We ask that you have it delivered to the Nurses Office before June 1st of Grade 6.
2. In Grade 7 the state requires us to document a current physical report. This report can be dated from March of your child's sixth grade until May of seventh grade.
3. There are no requirements for grade 8 but if we have not received a seventh grade physical we will not be able to send your child's health record on to High School.
We ask that you notify us when your child is scheduled for these appointments so that we can limit the number of request we send to families.
